Columns on the sheet

- Patient / ref
- Drawn up (mL)
- Given (mL)
- Wasted (mL)
- Wasted (mg)
- How wasted
- Wasted by

Filling in the mg column
Wasted mg is wasted mL times the concentration. At 10 mg/mL, 0.3 mL is 3 mg; at 50 mg/mL it is 15 mg. The waste volume calculator does both steps.
Questions
Why record drawn and given, not just the waste?
Recording both lets anyone re-check the subtraction later, which is the fastest way to find a transcription slip.
Does this tell me whether my log is compliant?
No. We keep and total your records; we do not give legal or compliance advice. The calculator adds and subtracts the figures you type and shows where two of them differ.
