Biennial inventory count sheet

A blank sheet for counting stock on hand, one line per drug, strength and form. Set the number of rows and the date, then print. 21 CFR 1304.11(c) says a registrant "shall take a new inventory of all stocks of controlled substances on hand at least every two years" (eCFR, 21 CFR 1304.11). What an inventory has to contain is set out in that section; this sheet is a counting layout, not a statement of those requirements.

Columns on the sheet

Totalling a line

Total mL is sealed containers times container size, plus the mL left in an opened container. Total mg is total mL times the concentration.

LineSealedOpened (mL left)Total mLTotal mg
Example A, 10 mL, 10 mg/mL46.34 x 10 + 6.3 = 46.3463
Example B, 20 mL, 50 mg/mL211.82 x 20 + 11.8 = 51.82,590
Example C, 50 mL, 100 mg/mL101 x 50 + 0 = 505,000

Before you count

Bring each container's running balance up to date first, so every count line has a computed figure to compare with. The running-balance calculator works out the computed balance and the difference from a count for a single container.

Questions

How often is the biennial inventory taken?

21 CFR 1304.11(c) says at least every two years, and that it may be taken on any date within two years of the previous biennial inventory date.

Does this tell me whether my log is compliant?

No. We keep and total your records; we do not give legal or compliance advice. The calculator adds and subtracts the figures you type and shows where two of them differ.
